A Case of Unilateral Posterior Reversible Encephalopathy Syndrome Occurring after Carotid Artery Stenting

چکیده
Objective: A case that developed unilateral posterior reversible encephalopathy syndrome (PRES) after carotid artery stenting (CAS) is reported. Case Presentation: The patient was a 79-year-old man who underwent CAS for symptomatic right internal carotid artery stenosis. Left hemiparesis and unilateral spatial neglect appeared on the day after the procedure. MRI presented findings characteristic of PRES although unilaterally. Temporary disruption of the blood–brain barrier due to blood pressure fluctuations in chronic unilateral hypoperfusion caused by stenosis of the right internal carotid artery or toxicity of the contrast medium was suspected. Conclusion: PRES is considered to be a complication worth attention in CAS.

Posterior reversible encephalopathy syndrome (PRES) is associated with various clinical manifestations such as headache, blurred vision, confusion and tonic-clonic convulsion. Some of the predisposing factors for PRES include hypertensive encephalopathy, preeclampsia and eclampsia, lupus erythematosus, thrombotic thrombocytopenic purpura and long-term use of immunosuppressive drugs. This condit...
متن کاملAtypical Unilateral Posterior Reversible Encephalopathy Syndrome Mimicking a Middle Cerebral Artery Infarction
Posterior reversible encephalopathy syndrome (PRES) is usually a reversible clinical and radiological entity associated with typical features on brain MR or CT imaging. However, the not-so-uncommon atypical radiological presentations of the condition are also present and they may go unrecognised as they are confused with other conditions. Here, we report a very rare case of atypical, unilateral...
متن کاملCarotid angioplasty and stenting in a patient with high grade stenosis of Internal Carotid Artery associated with both vertebral arteries and contralateral carotid occlusion
Severe internal carotid artery (ICA) stenosis is a common cause of cerebrovascular accident (CVA) in middle-aged patients. Contralateral carotid occlusion (CCO) in patients with severe ICA stenosis is associated with high risk of CVA. Carotid endarterectomy (CEA) is associated with more complications in patients with CCO than those without CCO. In this study, we present the case of a 61-year-ol...

Background: Carotid artery stenting (CAS) has recently been recommended as an alternative to carotid endarterectomy (CEA) by some clinicians. Objective: This study was designed to evaluate the success rate and in-hospital and 30-day adverse events in our first experiences in Iran for CAS with protection devices, to document our results and guide further use of CAS. Methods: From December 2...
متن کاملComments on "Unilateral Reversible Posterior Leukoencephalopathy Syndrome after Coiling of an Aneurysm"
BACKGROUND Reversible posterior leukoencephalopathy syndrome (RPLS) is radiologically characterized by symmetrical subcortical areas of vasogenic edema that are preferentially parieto-occipital, and it typically resolves after appropriate treatment. CASE REPORT We present a patient with strikingly unilateral RPLS that developed 21 days after coiling of an anterior communicating artery aneurys...
